When any patient comes to Crestwyn seeking treatment services, our staff conducts a comprehensive assessment in order to determine which program will best meet the individual’s needs. This assessment will give our team the background information we need not only to determine if we can provide the specific type of care the patient requires, but also to prepare an appropriate treatment plan should the patient be admitted into the program.
In addition to this assessment, our staff will also provide prescreening labs to ensure that the patient is medically capable of participating in programming. Should there be a significant medical concern, we will set up a transport to the nearest hospital. Crestwyn is in partnership with and in close proximity to a number of medical centers.
All admissions are determined on a case-by-case basis. Patients who receive care in our senior adult program must be able to actively engage in programming and have certain physical capabilities, such as being able to reposition themselves. However, we are able to make accommodations for various physical limitations, which can be discussed at the time of admission. Patients who are in the late stage of dementia may not be able to receive care at our facility, but that will likewise be reviewed at the time of the admissions assessment.
